This Intensive English Program Application will ask you to upload the following documents required for the I-20:

  • Passport Identification Page
  • Bank Letter

You may also be asked to submit additional documents depending on your situation:

  • Affidavit of Support 
  • Letter from Sponsoring Government or Organization
  • For dependents, Passport Identification Page
  • Disciplinary History 

 PDF format is preferred for uploaded documents.  Uploading the required documents as you complete this form will result in faster processing of your application.  If you are unable to upload them at this time, you may submit them later.

Please note that your application is not complete until you have submitted all required documents and have paid all required fees.  You can pay the application fee and express mail fee (if applicable) at online payment site.
